Mary plays her psychological insights/diagnosis of 'Donny' with a very straight bat. By the book. Most of us can deal with and do something about abandonment issues, drug dependence, stunted emotional growth ...and so on, pretty well. But the diagnosis by Mary of uncle Donny's narcissism is not just narcissism, most people have some of this as a survival mechanism. But nothing to the same degree than when this narcissism is to the level of a 'disorder.'
What she notes about her uncle is far worse than just 'narcissism.' She notes his condition as pathological, as presenting as NPD and/or BPD... Narcissistic Personality Disorder/Borderline Personality Disorder... you could probably throw in Antisocial Personality Disorder (Sociopathic) as well. The difference between being 'narcissistic' and the before mentioned disorders is like comparing a bit of sciatic pain in your leg to being paralyzed from the neck down!
Those folks with the above disorders, especially the 'white-collar' versions, present as highly charming, even warm and friendly. Even smart! They tend to speak in sweeping generalizations, be highly grandiose and seductive, convincing liars (they actually believe their lies) and everything and everyone around them is but an item for use - for them. Once their usefullness has been fulfilled, they're discarded. Yer fer me or agin me. Key word - me. Nutshell: very sad, and very dangerous. Tend to destroy all around them before destroying themselves. Their kryptonite? Accountability and facts.
